NOTE REGARDING MAILING OF YOUR PASSPORT(S)

Passport Services will not mail a U.S. passport to a private address outside the United States or Canada. If you do not live at the address listed in the "mailing address", then you must put the name of the person and mark it as "In Care Of" in item # 8. If your mailing address changes prior to receipt of your new passport, please contact the National Passport Information Center.

If you choose to provide your email address in Item #6 on this application, Passport Services may use that information to contact you in the event there is a problem with your application or if you need to provide information to us.

You may receive your newly issued passport book and/or card and your returned citizenship evidence in two separate mailings. If you are applying for both a U.S. passport book and passport card, you may receive three separate mailings; one with your returned citizenship evidence, one with your newly issued passport book, and one with your newly issued passport card.

FEDERAL TAX LAW

Section 6039E of the Internal Revenue Code (26 U.S.C. 6039E) and 22 U.S.C 2714a(f) require you to provide your Social Security number (SSN), if you have one, when you apply for or renew a U.S. passport. If you have never been issued a SSN, you must enter zeros in box #5 of this form. If you are residing abroad, you must also provide the name of the foreign country in which you are residing. The U.S. Department of State must provide your SSN and foreign residence information to the U.S. Department of the Treasury. If you fail to provide the information, your application may be denied and you are subject to a $500 penalty enforced by the IRS. All questions on this matter should be referred to the nearest IRS office.

NOTICE TO CUSTOMERS APPLYING OUTSIDE A DEPARTMENT OF STATE FACILITY

If you send us a check, it will be converted into an electronic funds transfer (EFT). This means we will copy your check and use the account information on it to electronically debit your account for the amount of the check. The debit from your account will usually occur within 24 hours and will be shown on your regular account statement.

You will not receive your original check back. We will destroy your original check, but we will keep the copy of it. If the EFT cannot be processed for technical reasons, you authorize us to process the copy in place of your original check. If the EFT cannot be completed because of insufficient funds, we may try to make the transfer up to two times, and we will charge you a one-time fee of $25, which we will also collect by EFT.

FEE REMITTANCE

Passport service fees are established by law and regulation (see 22 U.S.C. 214, 22 C.F.R. 22.1, and 22 C.F.R. 51.50-56), and are collected at the time you apply for the passport service. If the Department fails to receive full payment of the applicable fees because, for example, your check is returned for any reason or you dispute a passport fee charge to your credit card, the U.S. Department of State will take action to collect the delinquent fees from you under 22 C.F.R. Part 34, and the Federal Claims Collection Standards (see 31 C.F.R. Parts 900-904). In accordance with the Debt Collection Improvement Act (Pub.L. 104-134), if the fees remain unpaid after 180 days and no repayment arrangements have been made, the Department will refer the debt to the U.S. Department of Treasury for collection. Debt collection procedures used by U.S. Department of Treasury may include referral of the debt to private collection agencies, reporting of the debt to credit bureaus, garnishment of private wages and administrative offset of the debt by reducing, or withholding eligible federal payments (e.g., tax refunds, social security payments, federal retirement, etc.) by the amount of your debt, including any interest penalties or other costs incurred. In addition, non-payment of passport fees may result in the invalidation of your passport. An invalidated passport cannot be used for travel.

USE OF SOCIAL SECURITY NUMBER

Your Social Security number will be provided to U.S. Department of Treasury, used in connection with debt collection and checked against lists of persons ineligible or potentially ineligible to receive a U.S. passport, among other authorized uses.

NOTICE TO APPLICANTS FOR OFFICIAL, DIPLOMATIC, OR NO-FEE PASSPORTS

You may use this application if you meet all of the provisions listed on Instruction Page 2; however, you must CONSULT YOUR SPONSORING AGENCY FOR INSTRUCTIONS ON PROPER ROUTING PROCEDURES BEFORE FORWARDING THIS APPLICATION. Your completed passport will be released to your sponsoring agency for forwarding to you.

PROTECT YOURSELF AGAINST IDENTITY THEFT!

REPORT YOUR LOST OR STOLEN PASSPORT BOOK OR PASSPORT CARD!

For more information regarding reporting a lost or stolen U.S. passport book or passport card (Form DS-64), or to determine your eligibility for a passport renewal (Form DS-82), call NPIC at 1-877-487-2778 or visit travel.state.gov.

NOTICE TO U.S. PASSPORT CARD APPLICANTS

The maximum number of letters provided for your given name (first and middle) on the U.S. passport card is 24 characters. The 24 characters may be shortened due to printing restrictions. If both your given names are more than 24 characters, you must shorten one of your given names you list on item 1 of this form.

U.S. passports, either in book or card format, are only issued to U.S. citizens or non-citizen U.S. nationals. Each person must obtain his or her own U.S. passport book or U.S. passport card. The passport card is a U.S. passport issued in card format. Like the traditional U.S. passport book, it reflects the bearer's origin, identity, and nationality, and is subject to existing passport laws and regulations. Unlike the U.S. passport book, the U.S. passport card is valid only for entry at land border crossings and sea ports of entry when traveling from Canada, Mexico, the Caribbean, and Bermuda. The U.S. passport card is not valid for international air travel.

ELECTRONIC PASSPORT STATEMENT

The U.S. Department of State now issues an "Electronic Passport" book, which contains an embedded electronic chip. The electronic passport book continues to be proof of the bearer's U.S. citizenship/nationality and identity, and looks and functions in the same way as a passport without a chip. The addition of an electronic chip in the back cover enables the passport book to carry a duplicate electronic copy of all information from the data page. The electronic passport book is usable at all ports-of-entry, including those that do not yet have electronic chip readers.

Use of the electronic format provides the traveler the additional security protections inherent in chip technology. Moreover, when used at ports-of-entry equipped with electronic chip readers, the electronic passport book provides for faster clearance through some of the port-of-entry processes.

The electronic passport book does not require special handling or treatment, but like previous versions should be protected from extreme heat, bending, and from immersion in water. The electronic chip must be read using specially formatted readers, which protects the data on the chip from unauthorized reading.

The cover of the electronic passport book is printed with a special symbol representing the embedded chip. The symbol port-of-entry areas where the electronic passport book can be read.

How to Fill Out DS-11 U.S. Passport application

For individuals aiming to obtain a U.S. passport for the first time, or for those under 16 seeking to renew their passport, the DS-11 application form is the required document to begin the process. This form, while comprehensive, can be navigated with careful attention to detail. Completion of the DS-11 form is a crucial step in ensuring a smooth application process. It's important to understand that this form must be filled out with accurate and current information, as it serves as the foundation for your passport application. The following steps provide a guide for filling out the DS-11 U.S. Passport application form. This guide aims to make the process more manageable and less daunting.

  1. Locate and download the DS-11 form from the U.S. Department of State website. Ensure you have the latest version of the form.
  2. Read the instructions carefully before you start filling out the form. This will help prevent common mistakes.
  3. Fill out the form using black ink. Ensure that your handwriting is legible to avoid any delays in the processing of your application.
  4. Start with Section 1, providing your full name, date of birth, and place of birth as it appears on your birth certificate or proof of citizenship.
  5. In Section 2, add your Social Security number. Note that failing to provide this information may result in processing delays or denial of the passport.
  6. Proceed to Section 3 and provide additional personal details, including your gender, mailing address, and contact information.
  7. Section 4 requires information about your parents or guardians, necessary for verifying citizenship and identity, especially for applicants under 16.
  8. Provide your physical features, such as height, eye color, and hair color in Section 5. This information assists in personal identification.
  9. In Section 6, detail your occupation and employer. This is used for background checks and verification processes.
  10. Section 7 asks for emergency contact information. Provide details of someone who can be reached in case of an emergency.
  11. If applicable, fill in your travel plans in Section 8, but note that this section can be left blank if you have not made any travel arrangements.
  12. Ensure that Section 9 includes information about any previous U.S. passports issued to you, including when and where they were issued.
  13. Review your answers carefully for accuracy and completeness. Any mistakes or omissions could delay the processing of your application.
  14. Do not sign the form until instructed to do so by an authorized passport acceptance agent. Your signature needs to be witnessed as part of the application process.
  15. Gather all necessary documents, including proof of citizenship, a photocopy of identification, and a passport photo, to submit along with your DS-11 form.

After completing these steps, bring your DS-11 form and all required documents to a designated passport acceptance facility or agency. Here, an acceptance agent will review your documents, collect the applicable fees, and witness your signature. Timing for processing applications can vary, so it’s advisable to apply well in advance of any planned travel. Accurate completion of the DS-11 form is a significant first step towards receiving your passport and embarking on your travel plans.

More About DS-11 U.S. Passport application

  1. What is the DS-11 form used for?

The DS-11 form is the official document required for all first-time applicants applying for a U.S. passport. It's used by the U.S. Department of State to gather necessary personal information to process a new passport application. This includes first-time applications, replacing a stolen or lost passport, and applying for a passport for an individual under the age of 16.

  2. Who needs to fill out the DS-11 form?

The DS-11 form must be filled out by:

    • First-time passport applicants.
    • Applicants under 16 years old, regardless of any previous passport.
    • Applicants 16-17 years old, in some cases, depending on previous passport ownership or parental consent issues.
    • Applicants who have a previous passport that is not eligible for renewal (e.g., issued more than 15 years ago, or the applicant was under 16 when it was issued).
    • Applicants whose previous passport was lost, stolen, or damaged.

  3. Where can I get a DS-11 form?

DS-11 forms can be obtained by:

    • Downloading the form from the official U.S. Department of State website.
    • Picking up a copy from a local post office, which may offer passport services.
    • Visiting a public library or a municipal government office that offers passport application forms.
    • Attending a passport acceptance facility or agency, which can provide the form and further assistance.

  4. How do I submit my DS-11 form?

The DS-11 form must be submitted in person at a passport acceptance facility or agency. This is required for all first-time applicants and those whose circumstances necessitate using the DS-11 over a renewal form. Locations where you can submit the form include certain post offices, libraries, and county clerk offices. Applicants are encouraged to make an appointment where necessary and to come prepared with the required documentation and photographs.

  5. What documents are needed with the DS-11 form?

When submitting the DS-11 form, you also need to provide:

    • Proof of U.S. citizenship, such as a birth certificate or naturalization certificate.
    • Valid identification, such as a driver's license or military ID.
    • One passport photo that meets the required standards.
    • Payment for the passport fee, which varies depending on the type of passport and processing speeds desired.
Additional documents may be required in specific instances, such as parental consent for applicants under 16.

  6. Can I fill out the DS-11 form online?

Yes, applicants have the option to fill out the DS-11 form online through the U.S. Department of State's official website. This online form filler will guide applicants through the form, ensuring all necessary information is provided correctly. Once completed, the form must be printed out and submitted in person, along with all the required documentation, at a passport acceptance facility. Remember, even if you complete the form online, in-person submission is mandatory.

  7. What are the next steps after submitting the DS-11 form?

After submitting the DS-11 form and all required documentation at a passport acceptance facility, the application will be processed by the U.S. Department of State. Processing times can vary, but standard service typically takes 10-12 weeks. Expedited service, which involves additional fees, can reduce this processing time to 4-6 weeks. Once your passport has been processed, it will be mailed back to the address you provided in the application. Applicants can check the status of their application online at the U.S. Department of State website.

Common mistakes

Filling out the DS-11 U.S. Passport application form is a critical step towards securing your passport for international travel. However, amidst the excitement and perhaps a bit of stress about planning for travel, it's easy to stumble over some common mistakes. Being aware of these pitfalls can save you time, money, and frustration.

  1. Not using black ink: A simple yet surprisingly common mistake is filling out the DS-11 form with ink other than black. The U.S. Department of State specifically requires that all entries on this form be made in black ink, ensuring uniformity and legibility under all conditions.

  2. Providing incorrect fee payment: The fee associated with your passport application can vary depending on several factors, including age, whether it's a first-time application or a renewal, and the processing time you've selected. Calculating this incorrectly can delay your application process.

  3. Signing the form too early: The DS-11 form needs to be signed in the presence of an authorized passport acceptance agent unless you are renewing by mail. Signing it beforehand can invalidate your application, requiring you to complete a new form.

  4. Omitting necessary information: Failing to complete every required section of the DS-11 form can lead to delays. It's crucial to fill out every field, even if the answer is "N/A" or "None", to show that you didn't overlook the question.

  5. Submitting improper photographs: Passport photos have strict requirements regarding size, background color, and the applicant’s appearance. Photos that do not meet these criteria will be rejected, which can delay the processing of your application.

  6. Using the wrong form: The DS-11 form is specifically for first-time applicants, minors, or individuals who cannot use the DS-82 form for renewal. Using the DS-11 form when you are eligible to renew with the DS-82 can complicate your application process.

  7. Incorrect document copies: When submitting your DS-11 form, you need to provide certain documents, such as proof of citizenship and identification. Failing to submit the correct form of these documents, or submitting copies that are not clear and legible, can result in processing delays.

By steering clear of these mistakes, individuals can streamline their passport application process. Paying attention to detail and following the guidelines provided by the U.S. Department of State ensures that your preparation meets the mark, paving the way for a smooth and swift passage through the application process.

Documents used along the form

When applying for a U.S. passport using the DS-11 form, several additional forms and documents may also be needed or encouraged, depending on the applicant's circumstances. These items are important for verifying the applicant's eligibility and identity, ensuring the process moves forward as efficiently as possible. Below is a list of commonly required or used forms and documents along with the DS-11 U.S. Passport application form.

  • Proof of U.S. Citizenship: This could include a U.S. birth certificate, a Consular Report of Birth Abroad, a naturalization certificate, or a certificate of citizenship to establish the applicant's U.S. citizenship status.

Dos and Don'ts

When applying for a U.S. passport using the DS-11 form, it is important to follow specific guidelines to ensure the process is completed correctly and efficiently. Below are lists of things you should and shouldn't do during this process.

What You Should Do:

  1. Ensure the form is filled out legibly and with accurate information. Any incorrect information can lead to delays or denial of your passport application.
  2. Use black ink when completing the form. This ensures that all the details are clear and readable.
  3. Provide evidence of U.S. citizenship. This can include a previous U.S. passport, a certified birth certificate, a consular report of birth abroad, naturalization certificate, or certificate of citizenship.
  4. Include a suitable passport photo that meets the U.S. Department of State’s requirements. It should be a recent photo, taken in the last six months.
  5. Make sure to report your Social Security number, as it is required by law. Not providing your Social Security number can result in processing delays or denial of the application.
  6. Sign the form in the designated space only after being instructed to do so by an authorized Passport Acceptance Agent if applying in person. Your signature is a critical part of validating the application.

What You Shouldn't Do:

  1. Don’t leave any sections blank. If a section does not apply, enter a response that indicates this, such as "N/A" for not applicable. Missing information can cause unnecessary delays.
  2. Do not sign the form before bringing it to the acceptance facility if you are applying in person. The form must be signed in the presence of an authorized agent.
  3. Avoid using correction fluid, tape, or staples on the form. Instead, if a mistake is made, it's best to fill out a new form to ensure clarity and readability.
  4. Don't attach your photos with a clip or stapler. The acceptance facility will have specific instructions on how to submit your photos properly.
  5. Avoid guessing on dates or names. If uncertain about specific details, it's better to verify the information first to avoid submitting inaccurate information.
  6. Do not overlook the instructions for the supporting documents. Each supporting document has specific requirements and failure to comply can result in the rejection of your application.

Misconceptions

When it comes to applying for a U.S. passport for the first time, there are often misconceptions surrounding the DS-11 application form. Let's clear up some of these common misunderstandings to help streamline your passport application process.

  • Only for First-Time Applicants: Many believe the DS-11 form is solely for first-time applicants. However, it's also used by those who cannot renew their passport by mail, such as passports issued more than 15 years ago, those lost, stolen, or damaged, or if the applicant was under 16 when their current passport was issued.

  • Personal Appearance Not Required: A widespread misconception is that you can complete the DS-11 process entirely online. While you can fill out and print the form online, you must submit it in person if you're a first-time applicant, if your previous passport was lost, stolen, or damaged, or if you were under 16 when your last passport was issued.

  • Parental Information Is Optional: It's mistakenly thought that providing parental information on the DS-11 form is optional. In reality, applicants under age 16, or those 16 or 17, must include parental information to demonstrate parental awareness or consent for passport issuance.

  • Any Photo Will Do: People often think they can use any standard photo for their passport. The truth is, the U.S. Department of State has strict photo requirements regarding size (2x2 inches), background color (plain white or off-white), and various other factors. Non-compliant photos can result in application delays or rejections.

Understanding these aspects of the DS-11 U.S. Passport Application form can help make the application process smoother and more efficient. Always refer to the most current guidelines provided by the U.S. Department of State to ensure compliance and avoid delays.
