Common mistakes

When filling out the Navcompt 3065 form, commonly known as the Leave Request/Authorization form, individuals often make mistakes that can lead to delays or issues with the processing of their leave. Here are six common errors:

  1. Not following specific instructions for blocks 14 and 15 regarding start and end times: The start time must not be before the end of a normal workday if leave begins on a workday. Conversely, the end time should not extend beyond the start of a normal workday upon return.
  2. Leaving out important personal details:
    • Failing to provide a complete Social Security Number (SSN) in block 3.
    • Omitting or inaccurately filling in the name or pay grade in blocks 4 and 5.
  3. Overlooking leave balance and leave used sections: Neglecting to accurately fill out the leave balance and leave used this fiscal year (FY) in blocks 17 and 18, which could result in an inaccurate accounting of leave days.
  4. Inaccurate or incomplete travel plans:
    • Not specifying the mode of travel in block 12 when traveling outside the United States (OUTUS).
    • Forgetting to indicate if leaving or returning to Area of Permanent Duty Station (PERMDUTYSTA) in and outside the Continental United States (INCONUS) is involved in blocks 11a and 11b.
  5. Not certifying financial readiness: Failing to sign block 22, thereby not certifying that the individual has sufficient funds to cover the cost of round-trip travel.
  6. Incorrect contact information:
    • Omitting or inaccurately filling in the duty phone number and leave address in blocks 9 and 20, potentially hampering the ability to maintain communication during leave.

Making sure to avoid these mistakes can help ensure that the leave request process is smooth and that the leave authorization is not delayed.

Documents used along the form

When processing a leave of absence in a military context, particularly for Navy personnel, the NAVCOMPT 3065 form, known as the Leave Request/Authorization, is often accompanied by several other important forms and documents to ensure a comprehensive approach to leave management. These additional forms and documents play vital roles in the administrative, financial, and operational aspects of managing leave within the military framework.

  • Leave and Earnings Statement (LES): This document provides detailed information on a service member's pay, entitlements, deductions, allotments, and leave balance. It is critical for validating the leave balance before processing a leave request.
  • Emergency Data Form (Page 2): Officially known as NAVPERS 1070/602, it contains essential information about the service member's designated beneficiaries and contact information in case of an emergency.
  • Travel Itinerary: This outlines the member's travel plans and transportation modes, important for approving travel related to the leave, especially for overseas travel or when leave involves official duties.
  • Medical Clearance/Immunization Records: Necessary for service members traveling overseas to ensure they have the required vaccinations and are medically cleared to travel.
  • Request for Authorization of Temporary Additional Duty Travel (TAD/TDY): If the leave involves temporary duty assignments, this request form is necessary for approval of the TAD orders.
  • Passport/Visa Documentation: For international travel, valid passport and visa documents are necessary and may be required as part of the leave authorization process.
  • Family Care Plan Certification (NAVPERS 1740/6): For service members with dependents, this document outlines care plans for dependents during the service member’s absence, ensuring the family’s well-being is maintained.
  • Command Authorization: This includes any additional endorsements or approvals from commanding officers specific to the individual's unit policies or operational requirements.

Each document serves a specific purpose, from ensuring the financial readiness of the service member for leave to managing the care of dependents. Accurately completing and submitting these documents alongside the NAVCOMPT 3065 form enhances the efficiency of leave processing, ensuring that all aspects of the service member’s leave are properly addressed and documented within military protocols.

Similar forms

  • The DA Form 31, Request and Authority for Leave, used by the Department of the Army, is akin to the Navcompt 3065. Both forms serve the primary purpose of requesting authorization for leave from military duties. They collect similar information such as the service member's personal details, the period of the leave, the destination, and contact information during leave. Additionally, both forms require approval from the commanding authorities and emphasize the need for the service member to be reachable and ready for potential recall.

  • The AF Form 988, Leave Request/Authorization for the Air Force, shares a common function with the Navcompt 3065, facilitating leave applications and documentation for service members. Like the Navcompt 3065, this form requires details on the type of leave requested, departure and return dates, and leave balance information. It also highlights the importance of command approval and keeping the service member accountable for their leave period.

  • A similar document within the Coast Guard is the CG-2025, Leave and Liberty Authorization. This form, like the Navcompt 3065, is used by service members to obtain official permission for leave. Both documents collect information regarding the leave duration, type of leave, and member's contact information during leave. They also stress the reporting requirements upon return from leave and the need to maintain communication with the command during the leave period.

  • The Standard Form 71, Application for Leave, used across various federal agencies, parallels the Navcompt 3065 in its core purpose of managing leave requests. Although not exclusive to the military, SF-71 and the Navcompt 3065 gather similar key details like the leave type, duration, and approval endorsements. They both serve to formalize the leave process, ensuring that it is officially recorded and properly sanctioned.

  • The Department of Labor WH-384, Certification of Qualifying Exigency For Military Family Leave, while not a direct equivalent, shares similarities with the Navcompt 3065 as it pertains to military-related leave. This form is used to document and certify the need for family-related leave due to a family member's military service. Both documents underscore the importance of detailed information and official approval to legitimize the leave period.

  • The USPHS Commissioned Corps Form PHS-1345, Request for Leave of Absence, is utilized by officers of the U.S. Public Health Service and bears similarities to the Navcompt 3065. Both forms are instrumental in requesting and documenting leave, requiring detailed information about the leave request, including the duration and purpose of the leave, and necessitate official authorization from superiors.

  • The OPM Form 630, Application to Become a Leave Recipient Under the Voluntary Leave Transfer Program, though focusing on a different aspect of leave — receiving donations from other employees — shares the administrative foundation of documenting leave with the Navcompt 3065. Like Navcompt 3065, the OPM Form 630 plays a crucial role in the formal process surrounding leave, emphasizing the necessity of accurate record-keeping and official approvals.

Dos and Don'ts

Filling out the NAVCOMPT 3065 form, also known as the Leave Request/Authorization form, is an important process for military personnel planning to take leave. Here are several do's and don'ts to consider when completing this form:

Do:
  • Review the instructions on page 2 of the NAVCOMPT 3065 form carefully to ensure you understand each section and its requirements.
  • Fill out the form in triplicate using a ballpoint pen or typewriter, ensuring all copies are legible.
  • Print or type the information required in blocks 1, and 3 through 21, leaving block 2 for administrative use only.
  • Be accurate with dates and times when completing blocks 14 and 15, adhering to command policies regarding the start and end times of leave.
  • Verify your leave balance using your latest Leave and Earnings Statement or the Commanding Officer’s Leave Listing to fill in blocks 17 and 18.
  • Sign the form to certify that you have sufficient funds to cover the cost of round-trip travel and acknowledge the leave conditions.
  • Submit the form in advance of your desired leave start date to allow for processing through official channels.
Don't:
  • Skip the privacy act statement without reading. It outlines the authority, purpose, and routine uses of the information you provide, as well as your rights regarding voluntary disclosure.
  • Leave any sections blank that are applicable to you. Incomplete forms may delay processing or result in disapproval of leave requests.
  • Choose times that conflict with policy for your leave’s starting and ending hours (blocks 14 and 15). Always align with command policy and normal workday schedules.
  • Forget to include your leave address and a contact phone number where you can be reached during leave (block 20).
  • Disregard the requirement to immediately return your original leave authorization upon returning from leave, as advised.
  • Misstate your leave balance or requested days. Accuracy is crucial to avoid discrepancies that could affect your pay or leave status.
  • Disposal of the leave authorization. It is your official leave authorization and should be kept in your possession at all times during leave.

By adhering to these guidelines, military personnel can ensure a smoother processing of their leave requests, aligning with regulations and avoiding unnecessary complications.

Misconceptions

  • Many believe that the Navcompt 3065 form can be filled out and submitted digitally, but it requires completion in either ballpoint pen or typewriter. This ensures clarity and legibility for processing.

  • There's a misconception that approval of leave does not require a control number. However, as stated clearly on the form, leave approval is not valid without a leave control number, which is crucial for administrative tracking.

  • Some think that the Navcompt 3065 form is only for long-term leave requests. In reality, the form covers various types of leave, including regular, sick, emergency, separation, and retirement among others, offering flexibility for different needs.

  • It's commonly misunderstood that once leave is granted, it's absolute. The form indicates that leave is subject to recall, emphasizing the importance of maintaining communication while on leave.

  • A false belief is that the leave balance automatically updates on the Navcompt 3065 form. Service members must obtain their leave balance from their latest Leave and Earnings Statement or their Commanding Officer's Leave Listing and manually input it.

  • People often think that the instructions for the Navcompt 3065 form are complex. While detailed, the instructions provided on page 2 of the form are designed to guide the applicant thoroughly and clearly through each step of the process. These steps include filling out specific blocks with personal and leave-related information, and understanding the leave policy, as well as the day of departure and return calculations.

  • There's a misconception that any type of leave can be requested with this form without the need to provide a leave address. In reality, specifying a leave address is mandatory because, without it, leave cannot be properly granted or tracked.

  • Lastly, some believe that submitting the Navcompt 3065 form is the last step in the leave process. However, the form specifies that upon return from leave, the original leave authorization must be returned to the designated office as directed by command, signifying the completion of the leave process.
